Many living in Butte & Glenn county are experiencing food insecurity, housing insecure, already experiencing homelessness, possibly disaster survivors, and many don't know where to begin to access resources.  It can seem very overwhelming to know where to start, when your whole world seems to be crashing in on top of you. 

Outreach to Outcomes has been created to help fill a need in our community, the development of a team focused on outreach and case management services specifically designed to increase access and availability of resources to underserved and vulnerable populations in primarily Butte and Glenn counties.  By assembling a team of knowledgeable and compassionate providers, Outreach to Outcomes aims at overcoming institutional and systemic barriers in a socially conscious manner while maintaining a client centered approach to achieving individual goals. 

The founding members of Outreach to Outcomes are working hard to create an organization that is made up of a diverse group of people who are representative of the community they serve. Outreach to Outcomes understands everyone needs support and encouragement, but not everyone has a support system to fall back on, and sometimes we as a community need to create that for them so that everyone can achieve happiness and wellness.  

Outreach to Outcomes is currently providing approximately 10-20 hours a month of volunteer case management and administrative work in order to meet the needs of our existing clients and work to build the capacity of the organization. In order to apply for further grant funding the organization must have at least 3 months of funds in the bank to support reimbursable grant funding. By donating to Outreach to Outcomes you will be helping fund the seed money that we need to launch this organization and provide stability to our program, including hiring a part time case manager.  

By supporting the capacity building of this organization you will be helping 10 individuals this year with case management services and make an investment into our community. Outreach to Outcomes is a firm believer that 'people need people' to help them move forward in their journey and in turn we create healthy communities where all members are valued and supported.
